Understanding Cholesterol Using the “Highway Truck Model”


1. The Basic Idea

  • A building material for the body

  • Used to make cell walls, hormones, and bile acids

🚛 Lipoproteins

  • Trucks that transport cholesterol through the bloodstream

Triglycerides

  • Portable fuel containers carried by these trucks


The Lipid Transport System (The Highway Model)

🚛 LDL — Delivery Trucks

Carry cholesterol bricks from the liver warehouse to body tissues

Problem:
Too many delivery trucks → cholesterol traffic jam (artery plaque)

Target

  • Ideal: <130 mg/dL

  • High risk: >160 mg/dL


♻️ HDL — Recycling Trucks

Clean up leftover cholesterol from tissues & arteries and return it to the liver.

More recycling trucks = better cleanup.

Target

  • Men: >40 mg/dL

  • Women: >50 mg/dL


⛽ VLDL — Fat Cargo Trucks

Carry large boxes of triglycerides (fat fuel) from liver to storage.

Estimated value

  • VLDL ≈ Triglycerides ÷ 5

Target

  • <20 mg/dL


⛽ Triglycerides — Fuel Containers

Represent excess calories stored as fat fuel.

High levels mean too much energy circulating in the body.

Target

  • Ideal: <100 mg/dL

  • High risk: >150 mg/dL


⚠️ Lp(a) — Sticky Wheel Trucks

Special type of LDL with extra sticky tires.

They attach easily to artery walls and increase risk of heart attack and stroke.

Truck License Plates (Apolipoproteins)

ApoB — Delivery Truck Plate

All harmful trucks share the ApoB license plate.

Includes:

  • LDL

  • Lp(a)

  • VLDL

Counting ApoB tells us how many harmful trucks are on the road.

Target

  • Optimal: <80 mg/dL (high-risk patients)

  • Optimal: <90 mg/dL (general population)

  • High: >120 mg/dL


ApoA1 — Recycling Truck Plate

HDL trucks carry ApoA1 license plates.

More ApoA1 = more cholesterol cleanup.

Total Cholesterol

This is the total number of cholesterol bricks in circulation.

But the breakdown matters more than the total.

Target

  • Ideal: ~200 mg/dL

  • Both too high and too low may be problematic


The Big Picture

Healthy circulation means:

✔ More cleanup trucks (HDL, ApoA1)
✔ Less portable fuel (Triglycerides)
✔ No sticky wheel trucks (Lp(a))
